SERVICE CREDITS (SUBSIDY)
Service credits apply to the plans as follows:
¨ If Secure Horizons is elected, service credits will apply to the medical and prescription portion of this plan;
Otherwise
¨ Service credits first apply to the UnitedHealth Rx for Groups prescription drug plan (accompanies the AARP medical plans) UNLESS you “Opt Out” of the prescription drug plan.
¨ Any remaining service credits would then apply to the AARP Medical Plan elected. If you opt out of the prescription plan, all service credits will apply to the AARP medical plan elected.

Secure Horizons is a complete medical, prescription drug, and dental plan. You must reside within their service area in order to enroll (service area listing is enclosed). If you enroll and move out of the service area, you can make an application to enroll in one of the AARP plans.
Services are subject to co-pay. Only in-network physicians and facilities can be used. There is no out-of-network benefit.
Secure Horizons rates change each January 1st. Secure Horizons bills the member for any premium owed. The rate for 2010 is $235.51. For those with service credits, they apply to the medical and prescription portions of this plan; however, you must pay for the dental portion of this plan, which is $39.00. Retirees will be billed directly by Secure Horizons for the dental portion of this plan as well as any portion that exceeds their service credit allowance.

This plan provides coverage for all the basic benefits, including Medicare's Part A and Part B medical coinsurance. Rate changes apply April 1st of each year. Plan includes a discount drug card. United Group RX - Prescription Drug Coverage Information
This plan is available to those who select either of the AARP plans.
You are automatically enrolled in the prescription drug plan, called United Health RX for Groups, offered through United HealthCare which is an approved Medicare Part D prescription drug plan.
If you do not want to be enrolled in this coverage, you need to complete and return the enclosed “Form to Decline Retiree Medicare Prescription Drug Plan Coverage” (“Opt-Out” form) to us as soon as possible. The form is included in this packet. If you are eligible for Medicare Part D and decide not to be enrolled in any Medicare Part D prescription drug plan and do not have other coverage at least as good as Medicare prescription drug coverage, you may have to pay a penalty should you decide to enroll in a plan at a later date.
The cost of this plan for 2010 is $169.98 per month.

Unless you decline UnitedHealth RX for Groups coverage, you will receive a membership ID card, a Welcome Kit, and Evidence of Coverage in the mail. The Welcome Kit will contain a list of participating network pharmacies in your area and a Drug Formulary List.
